To start building your MyLife Health Record, you will need to collect your medical notes from your family doctor and other significant healthcare providers you may have. You may need to give them prior warning so they can organize copies to be made for you. They may be able to advise you on which parts of their patient record are likely to be useful to your future healthcare, or they may be willing to enter the information for you.
Physical copies will either be in the form of handwritten notes or printouts. Take your time reading through the notes before entering any information. After reviewing the notes some common threads may become apparent in your medical history.
You can navigate the sections of MyLife Health Record using either the left menu or by clicking MyLife Health Record - Update from your Personal Home Page.
Data entry forms are used on the site to input your information. You do not need to use medical jargon; there are spaces for your own words. If you are not sure of the correct name for a condition, enter your best guess then use the description boxes to enter as an explanation in as much detail as possible. Some parts of the record are linked to HealthAnswers web site to help you identify a disease or condition. Click the to search the corresponding category.
We suggest you start by entering your personal details and then complete the family history, exercise and activity history, surgical history, and medical history sections. You can upload any reports, investigations and x-rays you have in digital format.
Don't input all your information at once. You'll find the site easier to navigate on subsequent visits.
Your personal and health details are totally secure with us. Your information is encrypted in the same way that banks use to keep their clients data (and money) safe.
All information transfer takes place within your MyLife Health Record. No personal or medical information is sent through email. Email is only used to advise you about subscription information.
You access your record using a private User Name and Password that you nominate. In the interests in your privacy and confidentiality, we recommend you never share these login details with anyone else.
Your information will only ever be available to you and any clinician you deliberately grant access to.
